Verbal Consent Script

Onboarding Workflow Context:

Carrying a company-provided mobile device is a mandatory operational requirement for employment at Vincent’s Heating and Plumbing Inc.  During the initial technology provisioning and onboarding process, the supervisor or HR manager delivers the following verbal script to the employee before handing over the device:

Verbal Script Text:

"As a required tool for your job duties and workplace safety coordination, Vincent’s Heating and Plumbing Inc, routes internal operational updates, scheduling notices, and emergency alerts via text message to this company device. Do I have your permission to enable these text notifications on your corporate mobile number?

Please note that message and data rates may apply depending on carrier plans, and message frequency varies based on company operations. You can text STOP to opt out at any time if you do not wish to receive them, or text HELP for assistance. Because this messaging system is tied directly to the deployment of this company equipment, if you choose to opt out, we cannot issue you this company phone, and alternative operational arrangements must be discussed with HR. System Activation:

Once the employee verbally confirms "Yes" and accepts the device, the manager adds their assigned phone number directly to our automated email-to-SMS routing list to enable immediate outbound operational updates.
